Frequently Asked Questions
Paperwork doesn't have to be complicated. Below are answers to common questions about our exam process and requirements. For additional questions not listed here, please call our front desk.
Do I need an appointment for a DOT physical?
No. You can walk in for a DOT exam 24/7. Please bring your valid driver's license, any corrective lenses (glasses/contacts), and a list of your current medications to ensure you leave with your medical card the same day.
Do you have the sports physical forms?
We keep standard state forms on hand, but schools often have specific requirements. We strongly recommend bringing the exact physical form provided by your coach or school district to ensure 100% compliance.
Does insurance cover these exams?
Most insurance plans cover one annual wellness physical. However, DOT exams, work physicals, and sports physicals are typically not covered by insurance. When you visit we can confirm any covereage that your insurance may offer.
How long does a physical take?
Most wellness and sports physicals at The Heights are completed in about 20-30 minutes once you are in the exam room. DOT exams may take slightly longer due to the required urinalysis and vision testing.
